How I think about filler in 2026
The filler conversation has changed in the last few years. We have better products now, and we also have more patients who have been over-treated elsewhere and want to scale back. My approach is conservative: less product, placed more thoughtfully, with the goal of looking like a better version of yourself rather than like someone who has had filler.
The other piece is product matching. Lips need a filler engineered for motion. Cheeks need a filler with lift. Tear troughs need a filler made specifically for thin skin. One-size-fits-all fillers compromise on all of these.
The product lines I use and why
Restylane (Galderma) - The oldest HA filler family on the U.S. market and one of the most clinically studied. Strong in lips (Kysse), tear troughs (Eyelight), and structural cheek work (Lyft). The family includes seven distinct formulas, each engineered for a specific tissue behavior.
Evolysse (Evolus) - The newest family in my cabinet, FDA-approved in 2024. Made with cold-temperature manufacturing (Cold-X technology) that preserves longer HA chains. The result is a softer, more pliable feel in tissue. Strong for nasolabial folds (Smooth) and structural lift (Form).
What to expect at any filler appointment
Plan on 45 to 75 minutes total, depending on how many areas. We spend the first 15 minutes talking through what you want and what is realistic. The injection itself is 20 to 30 minutes. Topical numbing plus the lidocaine in the filler products keeps most patients comfortable.
Bruising and swelling are normal for 3 to 7 days. I see you back at two weeks to assess the settled result and add small touches if needed.
